Request Account Deletion

You have the right to request permanent deletion of your DocSlotBooking account and all personal data associated with it, in accordance with India’s Digital Personal Data Protection Act, 2023 (DPDP Act) and the SPDI Rules, 2011. Fill in the form below and we will process your request within 30 days.
1. What gets deleted
When we process your deletion request, we permanently erase:
- Your account and login credentials
- All patient profiles (primary + family dependents) on your account
- Health records, uploaded documents, and medical files
- Appointment history and consultation summaries
- Conditions, medications, allergies, and health tracking data
- Notifications and reminders
Exceptions
We may retain limited data where Indian law requires it (e.g. a minimum medical record retention period), and where another party (such as a hospital you visited) holds an independent copy under their own obligations. Anonymised or aggregated data that cannot identify you is not subject to erasure.
2. Deletion request form
Alternatively, email support@docslotbooking.com directly with the subject Account Deletion Request from your registered email address.
3. What happens next
- We acknowledge your request within 24 hours.
- We may verify your identity before proceeding.
- Deletion is completed within 30 days of verification.
- You receive a confirmation email once done.
